CPS Wellness Plan: What You Need To Know

The new CPS wellness plan keeps your healthcare costs downThe Chicago Lives Healthy Wellness Program is a comprehensive new wellness program for all employees and their spouses/domestic partners that are covered by CPS insurance. The goal for this program is to improve your general health and well-being to allow you to live a healthier life. Agreeing to this program also allowed us to freeze health insurance percentages in the new contract.

The initial program consists of a Biometric Screening and a Well-Being Assessment (WBA). The biometric tests consist of your sugar level, cholesterol, blood pressure, weight and BMI (Body Mass Index). It is recommended that you fast to provide the most accurate results. You do not need to fast if it is inconvenient or you have a medical condition that will not allow you to fast. All other tests and medical needs will be provided by your primary care physician. This program is not intended to replace your physician’s care or recommendations.

The biometric testing is free and available at the listed sites and you can sign up using this link or, if you prefer, you may go to your physician. If you choose to go to your physician and you list your visit as preventive care then it will be free. You may need to pay a co-payment depending on the terms of your insurance plan.

Upon registering you will need to fill out a WBA. This is a confidential questionnaire that will assess life and environmental factors that are critical to your health and well-being. It consists of 60 questions and should take you no longer than 20 minutes to complete. The answers you provide will NOT be shared with CPS.

Continued engagement is critical to the success of this program. When you have completed the one-time WBA you will then be required to participate in an ongoing interaction with Healthways. You will be asked to complete at least 15 minutes of wellness activities each month. Healthways will offer flexible online tools as well as telephonic health coaching and onsite activities aimed at helping you reach your health and wellness goals. You will NOT be subject to a penalty if you do not reach your goals. You will only be penalized if you do not log in monthly to participate in the coaching activities.

Healthways protects the confidentiality of all your information in compliance with the Health Insurance Portability and Accountability Act (HIPPA) which prohibits anyone from receiving your personal health information without your permission.
